Stock Request Form

🧾 Use this form to request additional supplies for your team.

Please fill in all fields accurately so we can track stock levels and ensure timely delivery.

✅ Requests will be reviewed by the admin team.

⏱ Urgent requests will be prioritized.

Vacuum bags should be used twice before disposal. Do not allow the vacuum bag to become over-full.

Cleaning Caddy

  • Upload 1–5 clear photos

  • Caddy must be open with all contents visible

Cleaning Equipment / Tools (PacVac Velo Go – IMPORTANT)

  • Upload photos of equipment regularly used

  • Include:

    • 1 photo of the vacuum head (top view)

    • 1 photo of the underside of the vacuum head

  • Photos are used to check for wear, debris build-up, and damage

  • Ensure photos are well-lit, in focus, and taken close enough to clearly show condition
